Term of Payment *

A deposit of $220.00 shall be secured on the day of registration to ensure a position will be held for participation in the EGC ISD program. The deposit ($220), if registration to the ISD program is withdrawn prior to September 15th, will be credited back to use towards any other service EGC provides (practice attendance, 1 on 1 lesson, game review, ect) or future development program (summer camp, spring program, ect). However, no credit shall be issued if application is withdrawn after the September 15th date. After the deposit ($220.00) is received, clients will have two payment options for the full remaining amount of the program ($2,300.00): i.) Clients may pay the remaining dues ($2,300.00) in a one-time installment due September 30th 2024; Or, ii.) clients may pay the remaining dues over the course of five equal monthly installments of $460.00, due on the first of each sequential month, starting October 1st, 2024, until the total dues are paid. Any late payment of an amount under this agreement will incur a 5% interest penalty per 7 days that payment is not received. By agreeing to these terms, clients agree that they are responsible for full payment of the program. In the event that a client wishes to cease engagement in the program, the client will be responsible to pay for all sequential ice times until the spot which was held for them is occupied.

In enrolling at Evolution Goaltending Corp.(EGC), the participant understands that he/she/they attending the programs and using EGC and the facilities does so at his/her/their own risk. EGC and its owners, employees or agents, shall not be liable for any damage whatsoever arising from any personal injury or property loss sustained by the participant with his/her/their family in or about any programs on the premises. Participants and parents/legal guardians assume full responsibility for all injuries and damages which occur in or about any programs on the premises, he/she/they do(es) hereby fully and forever release discharged hold harmless EGC., all associated facilities and its owner, employees, and agents from any and all claims, demands, damages or rights of action, present or future resulting from any person’s participation in any programs or use of the facility. In addition, he/she/they agree(s) to follow the rules of conduct and play set by EGC.. Failure to do so may result in suspension from participation. Consent: I the undersigned parent or guardian/participant do hereby grant authority to the staff at EGC. to render a judgment concerning medical assistance or hospital care in the event of an accident or illness during my absence. I do hereby authorize EGC. and its assigns to utilize any and all photographs, pictures or other likeness of me or anyone assigned guardianship to me, as they deem appropriate in its promotional materials or team films.
